The medical records department of The Family Health Centers can coordinate your medical records being sent to a new provider when you move or get you copies of your medical records to keep personally. Quite often, other medical facilities need copeis of patient records to help diagnose illness. Your signature is always required for us to release these records to anyone that is not exempt from our normal privacy policy procedures.
